Adult Foster / Family Care Overview
Adult Foster Care is a MassHealth-funded program that provides in home support to caregivers/ family members and AFC members and consumers. AFC members are individuals who are 16 years and older who need support in daily living. Support can be:
- Cueing and supervision
- Physical assistance with bathing, dressing, toileting, mobility, or eating.
- Behavioral management and support.
AFC caregivers are thoroughly pre-screened and qualified candidates. The AFC member and caregiver live together in a roommate or family environment which must pass inspection in order to be qualified. The staff is here to assist AFC members and caregivers with accessing program services. These services include:
- Assisting existing families through the process of becoming qualified
- Caregiver pre-screening
- Clinical and social member assessment
- Careful matching process
- Nursing and care management support
- Individualized training
- Plan of care to identify all goals and objectives to maximize care
- Monthly in home visits
- Information about community resources and other local programs
- Assistance with identifying alternate caregivers to help when your primary caregiver is not available
- 24 hour on call system.
Primary caregivers are paid a MassHealth tax-free stipend, while the AFC member pays room and board.
